A large percentage of patients with LBP > 4 weeks will not have symptoms of radiculopathy. The symptom of sciatica has such a high true positive rate for lumbar nerve root compression that its absence makes a clinically important lumbar disc herniation related to neural compression unlikely.
Reassessment is recommended at this time to seek a reason for the delay in expected recovery. After reviewing the patient's activity level and history and physical findings the clinician may consider further diagnostic studies.
