Ask Open-Ended Questions

Symptoms:

• Pain,numbness, weakness stiffness?
• Location: primarily in back, leg or both?
• Characteristic: constant or intermittent?

Limitations:

• Situation: sitting, standing, walking, lifting (how much)?
• Duration: How long?

History:

• How long have activities been limited?
• Previous episodes?
• Previous testing or treatment?

Expectations:

• What does the patient expect to accomplish during the visit?
